Thank you for selecting Advanced Orthopedic Center in Port Charlotte, Punta Gorda,  and Venice, for your orthopedic care. To schedule an appointment, please call us at (941) 629-6262 for our Port Charlotte or Punta Gorda office or (941) 497-1771 for our Venice office.

Once you have made your initial appointment, you will be given information to access our patient portal and set up your account. There you can pre-register for your appointment. Doing so prior to your visit will help speed up the new patient registration process.

Please be sure to bring your insurance cards and picture identification with you to your first visit along with any diagnostic films or other test results that may have been ordered or performed by another physician.

Established patients can update forms online through our patient portal. If you need help accessing the portal, please call the office and we’ll be happy to help.
